How to refer
Who can be referred?
Healthcare Professionals may refer any patient to the Hospice with a life limiting illness (e.g. Cancer, COPD, Heart Failure, Motor Neurone Disease, Dementia).
Patients can also be referred to our Outpatient Department for Chronic Pain Management either relating to a life limiting illness or other conditions.
Referrals are accepted for patients residing in the Tameside and Glossop area.
Who can Refer?
Any Healthcare Professional such as: GPs, Macmillan Nurses, Hospital Teams and Professionals, District Nurses, Long Term Condition Teams.
What can patients be referred for?
Services provided at the Hospice are:
- Inpatient Unit
- Day Services START Clinic which includes
- Dementia Support Service
- Physiotherapy
- Complementary Therapy
- Outpatient Clinic
- Physiotherapy
- Dementia Support Service
- Complementary Therapy
How are referrals accepted?
Each referral is assessed individually and discussed at our regular morning meetings (Monday – Friday), where the hospice team will decide which referrals are more appropriate to which service.
